If you donate stamps or other non-monetary items to LTDO, please use this form within 5 days of making the donation to submit a declaration of your In-Kind donation. I use these to report your donation to the State Board of Elections, like any monetary donation, as required by Illinois election law. You can get this form to me by:

● Returning it to me at a meeting or event.
● Scanning or photographing the completed form and emailing it to john-brubaker@sbcglobal.net.

This form is NOT actually turned in to the state, and you are not identified in my filing unless your donations, both cash and in-kind, exceed $150 for the quarter.
